Patients visit their GP for help with stress and symptoms related to mental illness.
Some of the issues that patients face during these times are caused or made worse by non-medical issues such as financial & employment concerns, housing issues, social isolation, substance misuse, bereavement or caring and parenting responsibilities.
We connect people over the age of 18 to community groups, activities and services in the local area who can help you with these issues.
We can see you for a period of up to 12 weeks to help you through a personalised plan of support.
