Annual General Meeting – Summary
We were delighted to welcome patients to our AGM on 27th November 2025. The session included updates on neighbourhood health plans, the NHS 10-year plan, and an interactive introduction to the different roles within our GP practice team.

Interactive Session – Meet Your GP Practice Team
- During the AGM, patients were asked to answer questions in the Teams chat. Here’s a summary of the roles and their responsibilities:
Mental Health Navigator (MHN)
Supports patients experiencing stress, anxiety, low mood, or emotional difficulties.
- Guides patients to talking therapies, community groups, crisis lines, and wellbeing programmes
- Often the best first contact for mental health guidance
Clinical Pharmacist
Your medicines expert:
- Reviews medications for safety and effectiveness
- Checks for side effects or interactions
- Provides advice and guidance on prescriptions
Advanced Clinical Practitioner (ACP)
Highly trained clinician who:
- Can assess, diagnose, and prescribe treatments
- Manages conditions such as infections, ongoing symptoms, and minor illnesses
- Works alongside GPs to ensure patients are seen by the right clinician quickly
First Contact MSK Practitioner
Specialist in joint, muscle, and bone problems:
- Assesses injuries, pain, and musculoskeletal conditions
- Provides exercises and management plans
- Refers to physiotherapy if needed
Why Knowing Your Team Matters
Using the right team member helps patients get b. Each role has specific expertise, so you receive the most appropriate support without unnecessary delays.
A message from our PPG chair Lucinda
The Patient Participation Group here at the Wells Medical Practice, is very vibrant and well supported. We aim to have an effective relationship with the Practice Team and to bring patient concerns, suggestions and compliments to the team in a supportive way.
To this end, we have regular meetings which are well attended by the group and the Practice staff. The shared professional experiences of the PPG are well received, and the Practice takes an interest in everything we have to offer that might benefit the wider patient community.
Our voice can bring your concerns to the Practice and we can make suggestions and discuss things that we feel will enhance the patient experience. For instance, the Practice is holding its second Menopause Morning Event on 16th January 2026. This is an activity that the PPG has backed from the start and been proactive in promoting... Recently, some of us were at the 'Vaccination Clinic' in October, where we helped guide you to the correct floor of the surgery. We also play a role in creating questions for the annual patient survey that is carried out by the Practice, and we are able to discuss and make suggestions about some of the issues raised by the results of the survey.
