My mHealth 

NHS Kent and Medway have been in partnership with my mhealth since 2020, with a primary focus on supporting patients living with Diabetes. The initial deployment has been a resounding success with over 13,000 patients enrolled onto myDiabetes platform.

We are delighted to share the exciting news that this partnership has now been expanded to include three additional long-term conditions; Asthma, COPD and Heart Disease (as well as continuation of access to myDiabetes).

my mhealth are ready to continue working with your practice to support patients living with COPD, Asthma, Diabetes or Heart Disease to get access to their self-management app. Our Digital Health Adviser team are also available to support your service.
