Join the practice
Prospective patients moving into our area may wish to apply to register with the practice. To do this it is preferable to bring along your medical card, which has your NHS number on it, enabling us to quickly process your registration. If you do not have your medical card there is an alternative form for completion, just ask the receptionist. You will need to bring with you some form of photographic proof of identity, such as your passport or driving licence. We will also need to see proof of residency; this could be a recent (within three months) bank statement or utility bill.

This message is to inform you of our boundary change application which we will be submitting. The below boundary map shows you which area we cover and will remain to cover. Abbey Court Surgery had a larger boundary area than Clanricarde Medical Centre did and as we continue to align services we are streamlining our catchment area so The Wells Medical Practice has one clear boundary. Thank you.

To be eligible to register at the practice, you must live within the practice catchment area.

Or you may print off a registration form and patient questionnaire, fill it in and bring it in with you to the Practice. These forms must be completed and added to the surgery’s records before any appointments can be booked. Please note new patients will be required to bring in ID and proof of address when registering.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Your Named GP
From the 1st of April 2015, the practice is required under the terms of the latest GP contract to allocate all patients a named accountable GP.
This is purely an administrative exercise in order that patients can have a named responsible GP should they require them.
For convenience your named accountable GP will be the named doctor you are registered with.
If you wish to be told the name of your accountable GP, please ask the receptionists when you are next in the surgery.
Please note: there is no need to telephone the practice for this information.
If you need input from a variety of health professionals, then you can discuss these with the named GP if you wish. Alternatively you can discuss these with any of the other doctors.
Having a named GP does not prevent you seeing any other doctor in the practice. Your named GP will not be available at all times and if your needs are urgent, you may need to discuss them with an alternative doctor.
The Wells Medical Practice can confirm that all of our patients (including children) have been allocated a named accountable GP.
